Statement Of Suspected Viloation
Every Employee Needs This Form.
Know as the Grievant Statement...

This form tells your Steward that you believe there has been a contract violation against you
or against the APWU. Fill this form out completely and give as many details as possible.
Dates, times, shift, names, work performed, violation observed and by who.
Once given to the Steward the investigation will begin.
You only have 14 days to fill this out and give it to your Steward,
otherwise it will be considered "Untimely" and the grievance is dead.

Stewards use this form to file a step one grievance on your behalf.
All APWU employees should take a look at this form and know where
the Steward will begin his investigation into a Grievance.
